Queenslanders are being urged to look out for this 4 year old boy who requires ongoing medical help.

A four-year-old boy has been found safe and well with his father after he was taken from Lady Cilento Children's hospital in Logan, south of Brisbane, on Thursday afternoon.

Queensland Police issued an amber alert on Friday for the then-missing boy, who suffers from a serious medical condition and needs ongoing medical attention.

The boy will be taken to John Hunter Hospital in Newcastle, authorities confirmed on Friday evening.

Police have not disclosed the name of the boy, who was seen being taken by his mother and father, despite being in need of medical assistance. Queensland Police confirmed later on Friday that the boy's mother was found in NSW, before the boy and his father were located.

There have been serious concerns for the wellbeing of the boy who is wheelchair-bound and cannot walk unassisted.
